Witness Statement template

During proceedings you may be asked to produce a witness statement. The court will give you a time frame for when you need to have this submitted by. This is a document which shows the evidence for the party writing the statement. This evidence is shown through exhibits. These can be things such as medical records, social services reports, text messages and email records. These documents should be labelled and attached underneath the statement with a schedule of exhibits attached to show clearly what is included in the statement. A statement of truth should also be included at the end of your document.

 

A template is attached for your use for your witness statement, it contains some guidance on how to lay out your stateme​​​​nt. Also attached is a schedule of exhibits template and exhibit template. 


Sometimes the court may suggest you use their C120 form, this is below.
